The Door Nobody Registered: Unregistered AI Agents and the Governance Blind Spot

EY survey reveals 26% of AI leaders cannot detect unauthorized agents. Why governance built for approved doors misses what's happening off the books.

I have a security system on my house. Sensors installed on every known door and window.

If someone cuts through drywall and opens a new one, the system doesn't glitch. It just has no idea that door exists.

EY surveyed 202 senior AI leaders this year. Board members and C-suite, all with direct oversight of AI governance at billion-dollar companies.

  • 98% have a formal AI governance policy.
  • 91% are already running agentic AI.
  • 26% can't detect an unauthorized AI agent operating inside their own company right now.

Same gap. The policy is real. The audit trail is real. But neither is watching for the door nobody registered.

Watching the Wrong Doors

You can build onboarding checklists for every AI tool that comes through official channels. But have you written one for the tool someone spun up over a weekend to hit a deadline? It never asked for permission because nobody made it ask.

Governance built to watch approved doors can't see the one someone quietly cut into the wall.

If an unregistered AI agent started acting inside your company tomorrow, who would actually notice?
